Do you say "sir" to people?

Out of respect ,like Indians do? What if it's someone who was knighted and actually has the "Sir" title like Sir Anthony Hopkins, Sir Elton John, etc.?

Another thing that I never understood is why American or British people insist with their titles, like in a movie someone asks for another guy's name: "Mister....?" and the other guy answers annoyed: "Actually it's doctor J. Random", I mean why do they insist on being called a doctor, captain, sir, etc., in my language we don't have this thing -, to brag with your job or title, everyone is a mister or miss, back in the communist era it was even simpler, everyone was a "comrade", you didn't have to memorize any other titles or even the names of people.
