I need CCW generally due to posteriorotation of the mandible, but maxfac gonna fix it with CCW on a maxilla or on the mandible? How does it work? AFAIK ccw on the maxilla should allow to fix downwarded grown mandible. Am I right?

you need CCW bimax, this is all you need to know. earn money and get shits done.

Any rotation involves both the mandible and the maxilla. It's called rotation of the maxillomandibular complex. You can't only rotate the maxilla without the mandible following along. This would lead to malocclusion. You can however rotate without advancing at the site of osteotomy.

If you have vertical maxillary excess, specifically anterior maxilla excess (gummy smile along the incisors) then you can do maxillary impaction which will automatically (autorotation) cause CCWr of the mandible without any need for BSSO.

If no vertical maxillary excess is present then you will need posterior downgraft of the maxilla which requires a BSSO for CCWr.

However, you can also "create" a gummy smile by undergoing a lip lift and then as a result of the newly created gummy smile undergo anterior impaction for autorotation of the mandible in a CCW direction.

I have underdeveloped maxilla which caused underbite and crossbite. I need a segmental lefort 1 according to my ortho, but I dunno should I get bsso also
