Over 18 they need surgery to open maxillary suture(and an appliance that screws into each side of the split suture) but yes, results are not much aesthetically it is mainly to help out airway (tho bimax is better roi for both aesthetics and airway)

The midpalatal suture fuses around late adolescence or early adulthood, so you'll need to look into a 3D CBCT scan to see where you're currently at and go from there. You can also look into MARPE, SARPE, or orthognathic surgery
