double jaw is for achieving perfect occlusion, you would be better with a jaw implant and corrective braces

Just claim sleep apnea and/or breathing issues and they should give you green light.

you can get jaw surgery even when your bite is fine, often you dont even need braces then

in some cases the surgeon might create an over/underbite before surgery depending on your measurements

It depends. If you have a steep occlusal plane and need a substantial CCW, then you can get a decent amount of chin advancement from the ROTATION of the maxillomandobular complex, apart from the mm of anterior mandibular movement along the bsso. If you have good occlusion and don't need a substantial rotation, then you can really only advance things so much without decompensating your bite as the mandible and maxilla will need to move in concert with one another to keep your current occlusion.
