Damn great news I will ask him next month on 2nd consultation my whole lower third is narrow and recessed compared to my neurocranium, would benefit a lot of chin wing

Nice man, I’m also having a consultation with Ramieri in the summer, I will be asking him lots of questions about the chin wing.

I’m having a consultation with Pagnoni in early April about possible BSSO, chin wing or genioplasty. Also may get implants with Liam at the same time.

Yes he does it but basically he doesn't like to work with it from what i have heard since it creates a "step off" in terms of transition.

Chin wing is more invasive genio when they also cut part of mandible close to chin and advance it with screws alone regular genio

BSSO is the regular mandible advancement performed during bimax

Ramus widening on condyle axis is osteotomy of the upper part of ramus giving you wider gonions

The chin wing isnt done that often because its outdated, any combination of BSSO, implants, genio will be better

The chin wing isnt done that often because its outdated, any combination of BSSO, implants, genio will be better
how surgery could be "outdated" lol? bsso and le fort techniques are 200 years old, are they "outdated" too? implants cause bone resorption and maintenance problems in future

You will look like Grishka Bogdanoff, it'll look unnatural, there is no way to prognose soft tissue movement a priori. Always risk of bone resorption with any material. The most fun will happen in your late life when bone in fact will get less dense and one morning you'll see your chin implant hanging from your left cheek. Also you'll never forget that you have some garbage under your skin, especially at the airport metal detectors. Ramieri and co. do it because its fast, easy and prominent result which he can post onto Instagram. Surgeons in Germany, Russia, Switzerland, Mexico, France performing chin wing, they are all wrong except Ramieri? "step off" is mitigated with bone grafts. CW solves problems of almost all 6-6.5 comrades (70% of the forum): increases ramus, narrows gonial angles, moves mandible forward.

Wrong if you come to giant. None of my 100+ jaw angles have been oversized except in 2 unnatural plastic surgery ken dolls who already came with oversized implants from somewhere else and wants to adjust them. So that effectively counts as none.

And no, Im not being biased because I make implants, I've been wanting to do a custom segmented chin wing for a while. I just know it's not the best solution compared to implants in 99% of cases. The results are less controllable & the surgery has way more risks, and it's not easy to revise when the patient (probably) realizes they made the wrong choice and wants revision/implants instead.

With implants you'll have them forever, with CW - 1-3 years max. Muscle is a tissue designed for being damaged, isn't it? As it sagged a little from cuts, it will recover back. Could you please explain in more detail why you think this is non-negligible issue? Still statistically you'll more likely to scar tissue replacing unsatisfying implants than CW / genio, e.g. see Gopnikcel thread, 3 surgeries with implants to just fallback for the CW

There is an opinion that cosmetic surgeons are mostly taught implants while maxillofacial surgeons taught osteotomy, hence the differences in approach. Nevertheless, all these surgeons I've mentioned have implants option and perform it on practice, but still they advice osteotomy.

I saw chin wing ops and papers, thats how they do it. For example 25:00, you can just google CW before / after, most of them don't have step-offs. I didn't get the rest of your phrase, what relapse and hardware failure are you talking about?

P.S. 3D model attached is the operation you performed? Looks like more than just CW but result should be amazing, do you have details on this op?

As of the information I was given a few years ago, which may now be outdated - the chin wing surgeons I consulted with told me they don't do implants anymore because of bone erosion they witnessed. This is just me repeating what they told me, as I'm aware this is a sometimes controversial topic. My limited understanding of implants is, bone erosion is rare in cheekbone implants, but more common with chin implants due to the amount of movement in the area. However I would defer to @RealSurgerymax on this as he has extensive real world experience.

In a nutshell, implants give more precise control about the shape and mass of the outcome. With a chin wing or other osteos you're limited by your existing bonemass and to some degree your existing shapes. Random example, you can do a chin widening osteo to try and get a more squared off chin, but in most cases it won't give you anywhere near the kind of squared off chin an implant will.

However, the big advantage of osteos which are unbeatable imo is how natural they look. You see a lot of implants and it just looks fake. Osteos look way more natural in comparison, if done well they're usually undetectable.
