Who is better between Raiffani and Z based on aesthetics reason ?
I need lefort 1 with bsso/genio and a ccw rotation for midface shortning. What would be the price for these ?

It’s hard to say without pics like I said but this would certainly reduce the length of the midface - just looking at him now it’s obvious he’s gonna need braces for a while too

ccw rotation for midface shortning

Counterclockwise rotation is not going to do anything for your midface length. If you have a Class 2 bite and a steep occlusal plane, which almost always comes with steep mandibular plane too, CCW rotation of the MMA complex is going to flatten it and bring overall better facial harmony. You won't end up with a shorter midface though because the work is mostly done in the lower jaw. However if you have excessive upper teeth show aka gummy smile which really is a long maxilla, you would be getting a LeFort I maxillary impaction osteotomy



You're saying though that you have an underbite and (unless you meant to say overbite) it's kinda rare to have a steep occlusal plane with Class 3 bite. Generally it's a good idea to post a few photos of what you look like but I understand not wanting to share pics with anyone here (and you shouldn't).


Both of the surgeons you're asking info about are more than capable of performing the type of operation you're interested in but if you're in the States I highly recommend you go to Arnett Gunson Facial Reconstruction in Santa Barbara, CA and book a consultation preferably with Dr. Gunson, though both of them are excellent. The wait times are long though, don't expect to see them this year if you phone their facility today. They will get you through a thorough evaluation, actually better than most maxfac surgeons.

You can't do much then, doing the type of osteotomy in the video on you would burry your teeth underneath your upper lip. You can "mask" the long midface appearance though with the following:

• upper lip lift if you have long philtrum
• cheekbone widening with either fillers or implants if you have a narrow face
• nasal tip rhinoplasty if you have a downwards rotated nose tip
• vertical jaw augmentation (implants or chin wing/sliding genioplasty) if your lower third is shorter than the other two

I wouldn't go to him, he charges significantly more than his work is worth, we're talking about 70k-100k here. Most of his results look uncanny valley too.
