what causes lines like this and having uneven fat distribution throughout his malar fat pads? ive seen many people with this issue and i doubt its a bone-related thing since many east asians dont get lines like this even with flat maxillas
there are also many obese people that still have good fat distribution all around.
i suspect its more fat distribution genetics but am i mistaken

is the solution to be lean?

are you talking about the nasojugal groove? a recessed maxilla can actually contribute to that combined with age-related descent of malar fat. but the line youre seeing in clav's pic is most likely just lighting/mild skin dehydration. also a mild line is common in everyone because the lower eyelid is thin and the malar fat is thick, which creates a junction/line

im talking more about the fat in the middle of his cheek causing the two lines across his cheeks. would slimming down your face fix this or would you need fat dissolvers?

no the medial fat pads in the middle of his cheek are not causing two lines, that line is a normal anatomical ligament attachment. if your medial fat pads were to atrophy it would have the opposite effect of what youre hoping would happen. the area under the line would lose volume, but since it is a tethered ligament it will not move down aswell, the crease will only look sharper on both sides. the usual fix is undereye filler/fat grafting
