With the second part of my comment, I’m getting at this: bimax is an overrated procedure. If you insist on getting it, get it in Italy. I think you’ll be dissapointed with the result bro. Rhino would do you so well.

I think in his case because its sort of a hyoid bone issue thats masking the definition, doing a slightly more specialised and assertive genioplasty might be able to counteract the affect a bit

Its fairly hard to understand why he didn't ascend.
If you check above the Lefort 1 osteotomy line in the green highlighted circles everything left behind.
The face is a continuum and it cannot be disrupted as it usually happens with Lefort 1 which is a really unnatural cut. The implants are far too high leaving a step off deformity and even if the implants extended lower it still wouldn't fix this aesthetic issue unless the projection was the exact same amount all across the length and height of the bone of upper maxilla (ie. 5mm advancement would need 5 mm implant projection ALL ACROSS the upper maxilla and infraorbital rims and not only on the apex of the zygoma) Lefort 1 is mainly 2D movement which affects the 3D projection in a very different way.
