Make sure to emphasize how important an aesthetic result is to you. Let the surgeon know you want to be very involved in the planning process. Ask the surgeon if it’s possible to use custom plates as well.
You’d prefer CCW over linear advancement, ask your ortho if it’s possible to talk with multiple surgeons, there’s surely a surgeon who’s willing to hear you out.

Yes, after you get your scans done. Get an online consultation with a good surgeon and explain your situation to them. You might have to pay for the surgical plan and consultation but it shouldn’t be expensive at all.

If the consultation is going to cost money email the surgeon beforehand and ask if it’s possible for them to make a surgical plan for you since some may refuse and you’d rather not pay for a consult for no reason.

The surgeon you get through insurance could also refuse but if you do everything necessary and the surgeon is capable of performing the proposed movements there shouldn’t be a reason for them not to follow the plan
