This is going to be a pretty quick thread.

The geniohyoid muscle is a muscle that creates part of the jawline. It is the band of muscle that anchors the hyoid bone to the internal surface of the chin. It has big implications in BSSOs. When the surgeon pulls the bone segment of the BSSO the geniohyoid muscle moves along with it like a dog on a leash, and sometimes upwards as other muscles (the stylohyoid suspension) become taught. The geniohyoid muscle in the annotated illustration above is "GENIO-HYIODEUS." This muscle has a fixed length. I think you can guess where this topic is going.

Geniohyoid sagging and surgical solution
Geniohyoid sagging is somewhat common I guess. It is caused when there is too much laxity in the geniohyoid muscle. It causes the jawline to be more like a jaw hill! Like the jawline bulges unasethetically downward. It can be fixed easily with a BSSO, as the movement causes the muscle to become taught and overcome gravity.

To validate the concept, you can see how the bulge underneath Bimaxer3000's profile shrunk when he got his BSSO. Bimaxer3000 fixed his geniohyoid sagging, mostly.


(In his results, it's still a bit there. But it's a solid improvement.)

Geniohyoid sagging exacerbates excessive fat deposits under the jawline.
