Diagnosed with midface deficiency. Since childhood I had an underbite where the front 6–8 upper teeth sat behind the lowers; over time, dental compensation caused my upper teeth to flare forward and my lower incisors to tip back, leading to lip strain and chronically dry, cracked lips. I also experience under-eye hollowing, dry eyes with prior infections, and difficulty maintaining nasal breathing, with a tendency to mouth breathe even during light exercise, leaving me feeling short of oxygen. My nasal passages become easily congested, and poorly ventilated environments worsen fatigue. I often wake from sleep or naps with severe headaches and daytime grogginess, raising concern for sleep-disordered breathing. Currently have a pending sleep test.

I wouldn't get advice from this forum but if you asked me I'd say nothing more than lf1 based on your description & pics.

you NEED a lefort III alongside 10 mm of mandibular advancement, genioplasty, and an OBO!
