Thats just facts, even if i talked some cuckery about him before. Some of the surgeons he works with is off. But he is the best implant designer out there by a long shot. Dont go to ram or pags designers, they are a lot worse

questionable attitude and behavior, not very professional, but yes I do think he is the best, he's dedicated to his craft, by how much he is better compared to other designers I don't think its necessarily by a long shot

perhaps but for the love of god don’t let him plan crazy osteotomies for you. i know an lf3 patient of his who got botched and giant did not care and was fucking random guys in thailand.

but also so many implants of his are getting misplaced. whether that is a surgeon problem or a design problem is hard to say. one surgeon i spoke with said it’s to do with the design

The infra implant results from ramieri aren't too bad, but they are from Facebook, what do you think @lurking truecel

What do you think about going to Liam for an osteotomy (bimax) rather than an implant?

no.

soft tissue is extremely important and a good surgeon will pay the utmost importance to it - by examining your face in person. without an in-person level of understanding of your face, any plan is not guaranteed to be 100% tailored to your actual aesthetic needs.

giant might have photos of your face, but chances are he hasn’t seen it in person before planning your surgery.

furthermore, he only works with shit surgeons who have numerous botches due to their poor surgical abilities. any self-respecting surgeon who is actually good at what they do would refuse to work with him (and i’ve heard stories about this).

besides, planning a bimax is not as much of a mystery as designing implants. arnett already wrote the golden paper on it, which revolutionised the field. raffaini is a student of his. many surgeons around the world use this analysis to some degree. this approach focusses on soft tissue landmarks rather than using reference points on the skull itself.

theres a lot of surgeons who prioritise aesthetics and you will get a far more reliable result with them rather than putting your fate in the hands of giant. focus on finding a good, trustworthy surgeon who puts aesthetics first.

yes it’s a brilliant watch.

arnett was the GOAT and gunson is from the same school of thought but less skilled. sadly dr arnett is retired. also HA paste for cheekbones is a fucking meme, but everything else he says is based.

what was the botch about
not 100% sure. the poor guy was an lf3 patient with celal/giant. all i heard was that he got botched and he went radio silent. i hope he’s found peace with his situation. giants lf2+implant patient also had loads of complications but i don’t remember what happened exactly. giant wasn’t bothered.

not anymore. his best results are from 10+ years ago. combine that with his demeanour, where you aren’t allowed to question or give feedback seems like a recipe for disaster, especially if you aren’t an italian foid which seems to be his favourite type of patient.

you should not rely on sites like these for good surgeon recommendations. DYOR and do it painstakingly.

yeah he is really good with foids and feminine looking guys. I noticed that.

if you dont rely on other peoples experiences you are pretty much left with consulting with random surgeons and hoping what they tell you in the consultation will yield good results. How can you be so sure about them? People getting botched by famous surgeons are more likely to talk about it online imo than with some unknown surgeon. you risk him having a lot of bad cases that nobody talks about online and he will just show you his best before afters obviously.

the issue is that these sites become echo chambers where only a handful of surgeons are discussed. and so the people on here only end up going to those handful of surgeons and it is a never ending cycle.

you’ll notice on r/jawsurgery how many LACOMS meatriders there are.

ramieri and pagnoni are not GREAT surgeons. ramieri is a good surgeon, and pagnoni is a good surgeon half the time and a butcher the other half. however, they are some of the most talked about surgeons on here.

there are so many surgeons beyond the common discourse of this forum who do brilliant work. but most people think: hmm, why would i go with these surgeons if i can’t find anyone who has talked about them online? so, they go to popular surgeons like ramieri or god forbid, pagnoni. the echochamber becomes stronger and good surgeons continue to stay obscure.

in an ideal world, the best surgeons quickly become the most talked about online. but everyone knows that’s not true. while you are right that other people’s experience on here is important, is there really any surgeons on here who every patient has spoke about positively? ramieri perhaps. but he’s conservative so botches are less likely and there isn’t always significant aesthetic changes.

i encountered some of my favourite surgeons after seeing a niche reference to them on reddit or on here, and id methodically check their instagram. i’ve got an instagram page dedicated to following good surgeons, and as such the feed just recommends me more surgeons. some good, some bad. theres so many surgeons who aren’t talked about with instagram pages of countless great results.

are u getting a bimax?
yes.

just posted a thread on HA paste, its safer yes but its mear balancing of the face, and from what ive seen gunson only places them submalar, I am currently about to start Invisalign in a few weeks that's based off of ramieris design but I am seriously considering switching to gunson. I want malar genio and jaw implants I figured I would do a conservative design with ramieri to minimize risk but im second guessing, What do you think I should do, go with gunson or ramieri? Im not crazy recessed, I am a sfs deep bite patient but not very severe and my cheekbones are high but lack forward projection jaw angle is good is not prominent

You can’t just switch to Gunson. You’ll have to wait 6+ months for surgery. And pay in excess of 60,000 USD. He’s good but he’s not worth the hassle.

Ramieri is quite mid but he will do a fine job, just not the best job possible. He can also do implants during the jaw surgery procedure which is a plus. However he doesn’t seem to be that confident with implants. Having said that, his recent small jaw angle results are good.

I used to tell myself I’d only go to Gunson and nobody else. But he’s not the holy grail and there are many surgeons who are similarly skilled without the hefty price tag and impossibility of obtaining a consultation.

I do have the money but its double the price to marginally improve the side profile, and it is true that although HA implants are safe, they are a meme, gunson always places them submalar and their only role is for balancing, not to actually improve the eye are or give better cheekbone definition, in fact ofter because they are placed submalar the patient looks like they have lower set cheekbones. its just extra hassle, and I already started the whole process, it is true that my cheekbones are already high but just lack forward projection, so I don't know maybe gun sons HA are perfect for me

The ortho Im working with is in close relations with him and Im expanding with marpe for now anyway, although Im doing Invisalign and marpe simultaneously. Still you are probably correct and I'll just take your word for it. and to be fair the jaw surgery result 80% has to do with your base and not necessarily the movements imo. once you correct malocclusion and maybe rotation all the extra advancement does is improve your side profile marginally

work in progress tbh im currently more focussed on orbital decompression and eyelid work.

i'm european. i've consulted with both bell and coceancig. i wouldn't recommend bell. the guy charges 90k to chimpmax you. you can definitely find capable and reasonably priced surgeons closer to you than italy lol. see my above comment about how these forums are echochambers of specific surgeons.

yeah I keep forgetting how expensive the surgeon near me are, I have a eu passport and have family in Rome, so I will still go to italy for the implants I guess, but yes these forums are echo chambers but ive already set everything up between ram and my ortho and and Im happy with what ramieri quoted me and ill get to vacation in italy while on some opiates :lul:

There is also this discussion online that surgeons like Walline and Gunson have a higher rate of asthetic results because of their similar analysis methods created by Arnett factoring in soft tissue norms. I guess Raffaini would then also use similar methods.

Except of having amazing before after results on an instagram page, how do you make sure that you are dealing with a great surgeon and not just a good one with some cherry picked results on his insta.

I guess there is a huge advantage if you first consulted with Gunson which will most likely give you a great analysis and recommendation what you actually need and afterwards you can compare every other surgeon you consult with based on what gunson told you. Does this make sense?
