So im starting the treatment plan for lower jaw surgery and the surgeon told me depending on the chin projection after the advancement he might also move the chin forward via genioplasty. I have a couple different options, I have to remove my lower premolars no matter what, since they said this is necessary for the advancement, but I can choose to keep my upper. My ortho said that keeping as much teeth as i can is the best option, so I think this is the route I will go. My wisdom will have to come out, you cant get BSSO with wisdom because the surgeon needs to make the cut there. Here are the surgeons notes: "Surgical 12mm BSSO adv. w/ yaw, EXT L4s".

But here is where I'm not sure what to do, they need to expand my upper arch somehow and some of my options are MARPE or dental tipping w/ AW expansion. They are trying to avoid having to do a Lefort 1 and make the expansion cut during the surgery as its much more invasive. I talked to both the ortho and the surgeon and they both said since I am past puberty, MARPE wouldn't do much - thats something thats needed to be done before 14. I am 20 now. The notes say "although dental tipping w/ AW expansion may be enough given lack of dental compensation". My teeth are really flared out already so they think its possible to pretty much tip them back downwards and just the expansion with the brace wires would be enough. Can anyone give some advice as to whether I should trust the expansion wires or really push them to include MARPE in my treatment plan.
