Surgeries can improve your base but they can’t fundamentally change it. Something that most users here don’t seem to understand.

At least 5mm of maxillary and mandibular advancement. Although more LeFort 1 advancement does not always equal better since you’re leaving the rest of the maxilla behind.

Even then, most of these cases will mainly have an effect on the side profile vs frontal.

The absolute best bimax results I consistently see involve downgrafting of the maxilla and mandible to treat SFS.

For SFS, the maxilla is not always hypoplastic so it depends.

I have a mildly “short face” and it isn’t due to lacking maxillary vertical height. It’s mostly from lacking chin height and a lower gonial angle.

I think a big step off heightening and forward advancing genioplasty and gonion implants will suffice in my case, due to having a relatively good bite already.

I remember seeing some very good ascensions from large genioplasties alone, from what I’ve seen Pagnoni is quite good at these big genios.

Any thoughts?

Yes, but he told me it varies patient to patient, depending on how much bone contact can be maintained when advancing forward.

Bone grafts/bone substitutes may be used in larger advancements too, Pagnoni told me he would use bone substitutes if needed.

A genio sounds like a good idea. Don’t restrict yourself to Pagnoni though; shop around locally first.

Doesn’t seem worth it to me to travel to a foreign country for a simple genio.

Yes, I live in the UK and have had multiple consultations, some surgeons dont do 3D planning still, which I am not comfortable with. Also many are somehow more expensive than Pagnoni, despite only usually doing a sliding genioplasty, not other modified cuts.

Also my local surgeons I’ve consulted have been reluctant to advance more than around 8mm, I’m not sure why.

I decided to go with Pagnoni due to his ability to advance the chin a lot and in different ways, also he does peek custom jaw angle implants, rather than basic silicone implants.

The way I looked at 2 weeks post Bimax compared to 3 months now, let alone what I’ll look like at 6-12 months, is literally a different human being.

I’m not going to “refrain” from anything.

I don’t have to personally experience something to have an opinion on it.

We have a hundreds of results online with people documenting their post op progress, swelling and all.

The guy had multiple surgeries over several years so I don’t know what “2 weeks post op” actually refers to here.

I assume the chin wing osteotemy, which was his last surgery.

The result for this kind of procedure is generally a marginal improvement at best.
It’s not going to re-invent the wheel

My instinct is that the changes will be incremental and I’ll stick with that.

If there are new photos which contradict this, I’d be happy to admit I was wrong.

now all he needs to do is bimax involving lefort impaction & genio impaction paired with revised wraparounds, as well as hairline advancement & getting rid of the zygo implats (or reducing them) - and he can finally start softmaxing!

it may look weird at first, as you switch between the pics (his traps look weird and it looks like hes getting squished jfl), but just zoom in and take a look at the morph of those surgeries alone and you will realize just how much it mogs the original

morph couldve been done way better, but im lazy & he tilted his head in such a way that it was really hard to fix the asymmetry (let alone his lips and moustache fucking with it too)

that would fix his fwhr, tfhrw, bigonial (cant be fucked to measure unmorphed, but it looks off) midface ratio, thirds, esr & jfa (fyi these ratios are literally the most important ratios on your face JFL)

Trimax with gonial implants would have been the best solution for him. Its not like surgeries wouldnt change much, its about what specific surgeries being performed on someone. Cheekbone expansion and orbital asymmetry reduction was a good choice, his big flaw now is his round undefined jaw which gives him this,,childish“ unmaskular look. Might also be swelling or he is just fat af
