MARPE expander with 4 micro-screws placed
0.25 mm per day (one quarter-turn in the morning, one quarter-turn in the evening) for 20–25 days until a 2–3 mm diastema

and begin wearing
the reverse-pull facemask with elastics (500 g per side) for 14 hours daily during sleep and after school

have the orthodontist place TADs (1.8 mm x 10 mm) in the infrazygomatic crest and mandibular buccal shelf. Attach nickel-titanium closed-coil springs (150 g per side) from TADs to the molar bands

herbst or forsus for mandible advancement 5mm protrusion

self-ligating braces on all teeth expect second molars
archwire 0.014" nickel-titanium heat-activated
in the first 3 months only NiTi wire
at the same appointment as braces placement have orthodontic insert 4 tads
Attach nickel-titanium closed-coil springs (150 g force per side) from each TAD to the molar hooks on the braces
