If your not lean get lean, you look like you have a layer of fat that you can shred off.

Your nose is your main issue, use accutane even if you don't have acne as it can shrink pores and can reduce your nose size. Use hydrocortisone cream to also shrink your nose, if you want stronger than use betamethasone.

After using accutane it might do something, but you can use hidrocortisone to shrink that area since your problem is a projected philtrum, there might be a surgery to correct it and since its skin and muscle its not that invasive and a simple fix
