IMO you need Advancement, it will 100% get covered if that X-ray is really you. The airway is fucked you are bound to get health issues along the road.

Surgeons actually prefer cases like yours because CCW without a down graft is the easiest variation.

You are bound to get sleep apnea/TMJ down the line, airway is very narrow.
