Help with ortho (pics)

My entire face is ruined by a completely undeveloped maxilla. When i was a baby my tongue couldnt fit in my mouth, all my baby pictures have it hanging out. They told my mom they could lower my palate and fix it all with surgery, but she thought it would hurt. So here we are, ugly af and unable to carry on the family name 25 years later.

I've got a meeting with an orthodontist coming up in a week. I think I can get double jaw surgery covered by insurance because i have breathing problems, tmj, and misaligned bite. Going to talk with them to see if they can deem it medically necessary. Any tips on approaching the meeting? Havent met the ortho yet so i dont know how they'll be if they want to hook me up or not. Ive seen on a lot of posts here leforte 1 and cheek implants are typically good enough for aesthetics.

My future plan is to also get rhinoplasty and a lip lift as i cant smile with my teeth, theyre too recessed. But I'm waiting for the maxilla surgery first as I hear it may upturn my downturned nose (I dont want to have them do that via rhinoplasty and then have double upturned nose). And also if the maxillary expansion shows more of my teeth, I'll need less of a lip lift, so if I do that too soon I could end up showing a bunch of gums in addition.

Any advice appreciated. Really dont think a basic palate expander will do the trick at this stage. I've attached a bunch of face pics, some selfies, where i naturally try to hide the smushed in midface, and some pics by other people where i cant cheat using angles. i just put a bunch so you'll see what im saying. the mid face needs some serious rework. already got a hair transplant 6 weeks ago, so no need for the hairline comments 🤣 itll be there eventually. I'll happily answer any gymmaxxing questions people have as those may come

Yes you need double jaw surgery and an infra-malar implant.

You will also need midface lift after that

Save rhino and lip stuff for after that as the jaw surgery will change your lips and nose

Okie dokie. added all this to my list. should have it all done in a years time or so, just depends on the wait times for the jaw surgery given that insurance covers it. in not i'll turkey it all.

Anything particular I tell the ortho or jaw surgeon? just to unsmush my face? 🤣

You need a lot of things honestly.

You definitely need a rhino after jaw surgery. Being critical here, your entire skull looks deformed especially in the midface to lower jaw section. Your lips have atrophied, all the surrounding muscles as well, making you look like an ogre.

You need djs, a rhino, honestly you're probably capped to a normie, don't know if its worth your time trying to fix these things when there isn't much potential.

Your best bet at love is finding a little ethnic asian woman and hoping you can at least let her use all your money for her companionship.

I am kinda in the same boat as you, but it's not this extreme.
You are recessed and you have SFS (short face syndrome). I don't know how your palate looks, but judging by the recession it's probably too narrow as well.
The mandible projection is fine, but chin projection overcompensates.

In case of a narrow palate, you will probably need to start with SARPE and braces to widen the palate and straighten the teeth. If the palate is wide enough, it will only be braces.
After a while of having your braces you will need trimax, not bimax. Let me explain why:

Your maxilla is severely recessed. It needs a lateral and medial movement (;forward and downward). This is to fix the recession and the short face syndrome.
However, with just a lefort it's hard to get a good bite, hence why your lower jaw will also need to move a bit forward to get a correct bite.
Here comes the catch: as your chin is already very projected, it will become a 'witch chin' if you just do a bimax with forward movement of the mandible. Therefore a slight genioplasty where the chin gets moved backwards and potentially slightly downward is also needed.

Lastly, I think once your recession/SFS is fixed, the rhino and lip lift won't even be necessary.

Get in contact with a good ortho and they can probably work out a plan with a good maxfac surgeon.

P.S.: genuine advice; work on your skin quality. You look old for being only 25.

thanks for the detailed breakdown. palate is definitely narrow, was hoping the jaw surgery would widen it. you think braces before the surgery is the right call? i was under the impression i'd definitely need them after, so i figured id just save them til after instead of doing two rounds

I think it's possible with a two or three piece lefort. My surgeon however recommended SARPE.
Braces are mostly part of jaw surgery. Might be different for everyone, but if your teeth aren't straight, you will most likely need it.

i was under the impression i'd definitely need them after, so i figured id just save them til after instead of doing two rounds
most of the times you first get your braces and have the braces on before, during, and after the surgery.
how long you have your braces again depend fully on your dental situation right now.

My recommendation (and from my surgeon) is to just get everything done correctly. Don't leave out things to save money or time.
Get a good ortho and maxfac surgeon and lay out the plan.

Last thing I want to say: getting jaw surgery is a whole process that takes months if not years.
You do it once and enjoy the rest of your life. Make the right decisions.

human shrek,i find you very handsome (hypermasculine look) the only ugly thing i see is that your nose is a bit too big,and use volufeline for fuller lips

🤣 ive been called human shrek before. ive definitely got masculine features, but whatever the opposite of harmony is i accidentally maxed that out 💀 im thinking the jaw surgery will change the nose and possibly the lips, so we'll see. I've been moving forward with this since the original post; ive had 2 consultations with an ortho. dude seems genuinely interested in helping and isnt just a salesman. Got a bunch of xrays and a few molds done and he's working with the jaw surgeon who's going to reach back out to me sometime next week. Ortho seems to think i can get sliding genioplasty covered to fix a good bit of the asymmetry too.
 
Double jaw surgery with clockwise rotation is the way to go. would fix your smile as well.

Ngl bro I usually hate it when people recommend bimax bc it just isn't warranted. In your case however, bimax would literally change your life, emphasis on the lefort cut.

I'd say that's the biggest possibly for you relative to attractiveness, bigger than your maxilla.

Been doing a lot of skincare since i posted this and I think I look more my age now. obv not a huge difference but it's only been a month. a step in the right direction for sure. my hairline will grow back in over the next few months and thatll knock a few years off too

Thanks. just hoping this maxillofacial surgeon knows what he's doing 🤣 the rest is light work once i get the skull shape fixed. ive got a really good job in an area with a really low cost of living so i can get the other cosmetic surgeries easily. just need the leforte first.
