During a filming delay of the tv show Cover Up Episode 7 Golden Opportunity
Jon Eric Hexum who had fallen asleep on set woke up begun playing russian roulette not knowing that when he was asleep the shows armorer had loaded it to make it faster when they went back to work
As he pulled the trigger the blank cartirdge left his temple hemorrhaging blood and he was rushed to hospital.
