How (and why) to minimize hyperphosphatemia on erdafitinib
This is a simple guide on how to minimize the root cause of what most people to consider to be the main side effect of erda: high phosphate, aka hyperphosphatemia, and why it's probably worse than you think.
TLDR: Increase erda results and combat it's sides by reducing your phosphate levels through binders, tenapanor, acetazolamide, cutting dietary and added phosphate, loop diuretics, and nicotinamide
First, why even care about high phosphate on erda?
A lot of people wrongly think that you can dnr high phosphate because all it does is cause things like joint pain, weakness, fatigue, etc which is very very wrong. High phosphate can actually impact your precious height growth through the process of
. In simple terms, hyperphosphatemia makes the cells that build your bone die more.
Also, phosphate is pretty easy to reduce at many benefits toward both height and your overall health. Hyperphosphatemia has an array of other health effects that I won't go over here because they are pretty well known, especially firsthand by erda users.
So how do you reduce it?
There are 8 easy ways to reduce phosphate to maximize the effects of erda, alongside helping to negate other health effects as well.
1. Running only 2-4mg erda
This is the one that will probably be the least desirable, but is the most effective. You can still get a good amount of erda's benefits while running only 2-4mg/day, which will substantially reduce high phosphate (as well as all the other sides obviously).
2. Phosphate binders
You can reduce phosphate by binding phosphate, aka increasing the amount that you shit (or piss) out. This can be achieve through an array of treatments:
- Calcium acetate [link] or indiamart
- Sevelamer (indiamart)
- Lanthanum (indiamart)
3. Tenapanor
Tenapanor is sorta expensive (from the research I did, maybe a cheap source exists), but it works by reducing the amount of phosphate that goes from the diet to the bloodstream.
4. Acetazolamide
This treatment has sides of its own, but mechanistically can reduce high phosphate by reducing renal retention, and making you piss out more of the phosphate.
5. Cut dietary phosphate
This one is sorta annoying because there's phosphorus in fucking everything, but if you're willing to eat mainly chicken and fruit during your erda journey then it works to an extent. You can do this easily by downloading apps used by dialysis patients (e.g. Kidney Pal) and selecting that you're on stage 2-3 kidney disease. The issue is that the kidney retains phosphate anyway, so you need to do this for a long time for the effect to start.
6. Loop diuretics
This has some minor sides (smaller than acetazolamide's) but also mechanistically can reduce high phosphate through increased sodium and water delivery and weak carbonic anhydrase inhibition.
7. Cut added phosphate
This one is sorta niche, but if you take some sort of vitamin D supplement or a protein powder that has phosphate salts, cutting it can help. This sorta ties into 5 but I isolated it because it only applies to people who are making their hyperphosphatemia worse without knowing it.
8. Nicotinamide
This is last for a reason, it has a weak effect on intestinal phosphate absorption, but still worth mentioning.
NOTE: You can combine all of these together safely EXCEPT combining acetazolamide and loop diuretics (as in, those 2 specifically should be mutually exclusive). You can fairly easily run all of these except for acetazolamide combined to nuke your high phosphate issues, but it might be a little expensive.
You can also follow this simple guide of what to do depending on your daily dose of erda:
<2mg: Probably nothing unless you get high phosphate related sides
2-4mg: Slightly cut dietary phosphate and forget
4-8mg: Run a phopshate binder and tenapanor
8mg-12mg: Cut dietary and added phosphate, run 1-2 phosphate binders, and tenapanor
>12mg: Everything other than acetazolamide
TLDR: Increase erda results and combat it's sides by reducing your phosphate levels through binders, tenapanor, acetazolamide, cutting dietary and added phosphate, loop diuretics, and nicotinamide
or just run tyra if ur rich lol
(All satire and hypothetical, I do not condone the use of prescription medication without a prescription)
