There is definitely a limit that Eppley himself postulated.
While a liberal plastic surgeon might say up to 20mm is the maximum for chin implant (with no vertical augmentation or with that being a combined anterior+vertical number), a more conservative one would say about 10 anterior or 12 combined.
There are 2 reasons I would say about 12mm combined is the advisability MAXIMUM:
1.) Natural looking soft tissue chin pad re-drape. When we say maximum I am not saying the absolute biggest you can go and still be able to close the incision and not have it rip open later. I am saying the maximum for a natural and improved soft tissue drape in MOST cases. I will admit I have seen some very impressive HUGE implant cases.
2.) Acceptable implant bone impression. Pressure Inside the implant pocket which the soft tissue envelope is exerting on the implant which it is transferring to the bone will cause a 1-2mm deep bone impression. This is not aesthetically noticeable and comparable to the relapse you get from osteotomies. But an 18mm chin implant is where bone erosion actually is a risk.
These are general statements. It depends on what type of chin deficiency one has as well. There are 4 main classifications but even more since many Co-exist At the same time.
