Despite the terrible photo, you can tell @Raf_slv10 would benefit from at least 3-4 surgeries. Namely :

> genioplasty (vertical augmentation)
> lip lift (potentially necessary to restore chin to philtrum harmony)
> otoplasty
> canthoplasty and/or a combination of oculoplastic procedures

His current chin to philtrum ratio is only 1.5.
His ears are definitely too flared.
His canthal tilt is at best neutral, and possibly a bit negative, which is visually emphasized by the droopy upper eyelid.

Chin shouldn't just be extended vertically like that it depends on other factors - his MPA also affects this so recommending him to do a vertical genioplasty can cause a bad step off regardless of bone grafts and cause aesthetic issues like ballsack chin

Eppley is smart as fuck as he knows about this so he combines it with implants to keep the mandibular border continuous but now you are just making it way more expensive and complicated

You also have to assess this with respect to the mentolabial fold which we can't guess from this front profile

Also any type of genioplasty focussing on advancement and height will make his chin narrower due to the viewing angle

Canthoplasty is shit they can't ever get it right show me one good result post 6 months. Look at facemetrics on YT (he's a fraud don't pay attention to his content) he had a canthoplasty and now he still has an NCT. Bridge of Bone canthopexy is best bet but it's permanent and can be fucked up too most results look botched with tension in the upper eyelid it looks horrible

Otoplasty his ears aren't even that flared it wouldn't even boost his SMV

this is just ratiocel behaviour sure I could measure everything and morph him but you gotta be realistic about this. Morphing doesn't have a solid place in this subforum if you don't respect the limits and realism of these surgeries

at some point you gotta realise if these surgeries are even worth it so it's better he doesn't bother he looks great anyway despite these flaws - it's why I said he doesn't NEED not that there isn't anything he can do and that's just based off this shit photo

based off his bottom lip Im guessing his mentolabial fold is kinda deep

Secondly explain how this fixes any of the issues I talked about

Nothing here indicates it can avoid Pharaoh's chin or reduce the step off

If you look at the diagram provided you can literally only go Anteriorly or Posteriorly rotation would cause a crazy step off - even worse than a genioplasty step off

Also I wanna see studies on this long term. No one should be inventing variants for surgery without studies to back it up - an example would be Dr Ramieri's Tetris genioplasty which he has published studies on to show the stability of it

I mentioned that i dont like the execution from dr. Safi in this case.

What are you on about it does avoid the "Pharaoh's chin" and with a better design also the step off regarding the mandible.

this kind of design is better imo

the gap in the chin can also be fixed with a custom designed hydroxyapatite implant or regular hydroxyapatite paste.
