How can I fix prognathism and downward growth?

I have pretty low gonial angle and very downward grown, on top of that I'm very prognathic and have to keep my head tilted down and my mouth open to hide it, how do you fix this? At least the prognathism if nothing else cause that ruins your whole face and makes your shit look a chimp.
