Those are hollow under-eyes. You can get a temporary fix with fillers, but they will move around after a year and the results will be completely gone. On the other hand, you can fix the root issue. A modified orbital box osteotomy will get you right.

You're right, it's a bit extreme for his case, since OBO should be reserved for IPD correction. Fillers are more realistic.
