How can i get smoother skin?

i need to know how to get smoother skin without using a fuck ton of chemicals or going to a dermatologist. currently i use a low strength face wash, retin A, and a water based moisturizer. i don't really need anything stronger cleansers since i don't have bad acne but my skin looks dry and my pores are visible. any time i have switched to a new moisturizer i have immediately broke out with acne. how can i just have a smooth, moisturized face without getting a shit ton of acne?
