The Empire did care about what Imperial citizens thought and said. The planets they controlled were all military police states with lots of propaganda and thought control.

There's an interesting YT video that explains the Emperor's ultimate ambitions and what he hoped to achieve, which was to essentially exist as a god for an eternity.

In that sense he didn't really care about his citizens, just they they didn't block his ambitions.

Some of the governers of various planets were weaker than others. Some had less resources to operate with. Some sympathised with the Rebellion over time. There was also alot of corruption.

However, Star Wars is Space Fantasy rather than hard Sci fi.

It's more about the stories and characters.

It's a bit like comparing Jazz music with Classical music.

I guess it's a bit like in the Warhammer 40k universe. For example, Space Marines are superhumans with like 2 hearts and much more powerful internal organs. They can health wounds that would kill regular humans. However they can still be cut down in battle and die.

Also, that type of medical care might not be available to everyone in the Star Wars Galaxy.

Again the logic in the SW universe is designed to be flexible, to further the stories. Like one moment they have super advanced tech and another barely have wireless technology lol.
