How do I fix my face

I have a bad bone structure and I tend to look bloated very easily, even at a lower bodyfat. How would I go about fixing this, and what type of doc can I ask these questions to? I used braces (an extraction + elastics) before.

yes i think it would, but I'm not knowledgeable in surgeries, I'd say bimax for sure though to start
