How do I get my double jaw surgery covered under insurance?

I'm extremely committed to getting a bimax within the next year or two. What are the specific requirements for it to be covered under insurance? My mandible is recessed, but I don't know how bad it has to be to get covered, could someone tell me some specific measurements the ortho uses to determine if I can have a double jaw procedure covered under medical insurance. I was also wondering what other problems can lead to it being covered I've heard that sleep apnea is a valid excuse for it.
