how do mental illness diagnoses make sense

i can understand things like schizophrenia being diagnosed but what about things like bipolar, bpd etc
a doc listens to the "patient" but wouldnt a diagnose only be possible in the case where the patient is able to perfectly express themselves which wouldn't be very possible
testimonies from people around them cant be that reliable neither innit????
