How do you know to have a good base for surgery?

I’ve seen around some incredibile results but can’t tell how to establish if someone has a good base.

For example I wouldn’t have said that Amnesia had a good base but apparently I am wrong. Mirin to him

It alls comes down to ratio since you might have a good base even without bone mass?
