I am going to get jaw surgery in the future and want to know what to tell the orthodontist

You don't tell the orthodontist anything. You find a good surgeon and they tell the orthodontist what needs to be done.

i was kidding, you're not death tier recessed but still very recessed.
probably like 15mm at the b point that means you most likely need big ccw rotation

no advancement since your nasolabial angle is ideal
His maxilla is also recessed imo. Both jaws need advancement on a Quick Look. You can also tell because his nasal tip is slightly droopy. , which is a sign of insufficient upper jaw growth.

Jaw surgery will mainly affect your side profile. It won’t address the under eye defiency. You’ll need implants for that. Your frontal view won’t change that much without it.


You look like you have a mild overbite to me. Am I right?

Thought so. Also downward growth. You’ll likely need upper and lower jaw advancement with counter clockwise rotation.

Perhaps a genio too but I warn you, your front won’t change that much without implants.

I'm not implying that his maxilla is not recessed, all I'm saying is that lefort 1 advancement would look bad.

It looks slightly to me. It would be more obvious if he smiles. The smile exposes the true position of the maxilla because soft tissue is pulled back.

OP take a picture of yourself smiling from side profile to prove it.

I think he needs both jaws advanced myself. His overbite gives the illusion of better upper jaw projection when both jaws are recessed imo

Yep I was right. OP’s upper jaw is also recessed. Needs LF1 for sure. Lack of bone support at the base of the nose causes it to droop significantly

No, it looks normal. The nose isn't supposed to stay the same when you smile.

Going to have to consult with several surgeons about this.

I can eyeball it but do not take this as gospel. Speak to professionals.

5mm LF1, and BSSO advancement to get 2 mm overbite (ideal).

Any more than 5mm and the rest of your maxilla looks left behind.

You’ll need counter clockwise rotation of maxilla to counter downward growth. How much I don’t know.

I don’t think it does myself. Of course the nose doesn’t stay the same. Never said that. The whole nasal tip bends into a 6 shape when he smiles though.

It’s not soft tissue imo - it’s lack of bone support. You can see that if the LF1 area were advanced it wouldn’t droop anything like as much.

I’m happy to be proven wrong. Any other users here who disagree with me?

OP speak to several surgeons. I’d bet all advocate some LF1 advancement

I advice save 15-20k and go to Ramieri

He is one of the best to fix recession for class 2 malocclusion or any other jaw deformity

You don't need that much forward growth to look balanced and good.

Around 9-13 mm BSSO + 5 mm genio
3.5 LF 1 with CCW
