Good base for surgery do a full rhinoplasty with osteotomy and also clear that skin you can probably get accutane prescribed (has some mild sides) also upper lip seems to super thin and quite a long philtrum so do a bullhorn lip lift and it will help to make the philtrum not look quite also will make your pink part the vermillion more visible.
