Bodybuilders tackle this problem often. I suggest to visit some roid forums, they know what will work. prednisone is for example used.

Acne is the same regardless of position.
However, here are some things you may try specifically in regards to chest/shoulders acne:
> change shirts more often, never keep the same shirt on you for more than 12h if you can
> don't wear tight shirts
> shower right after working out
> workout shirtless if possible
> don't shower with hot water. lukewarm/cold is better
> exfoliate gently
> buy an anti-acne bodywash

From my experience acne on the back was the most difficult to fix using retinoids and other creams - pretty much zero effects, BUT it was the easiest to get rid of on accutane - it completely went away after 2/3 months on accutane.
