Just find a local surgeon and complain about sleep apnea and try to steer the conversation towards getting a CT scan. Once you’ve the scan you can consult anyone you want.

If you can access scans without referral then even better, just go straight to that. Make sure it’s a good machine (320 slice) and that it can do your whole face and you’re good to go.

If you just want bimax you can likely get away with a dental CBCT scanner as well but generally they will tell you to fuck off.
