The buccal fat pad is a deep fat mass in each cheek that sits between the buccinator and masseter muscles.
Its one of the main soft tissue structures determining mid and lower face contour
larger pads create fuller rounder “baby face” or chipmunk cheeks

Its volume is largely genetic and relatively stable with body weight changes
over time it can atrophy or descend, contributing to age-related midface hollowing.

buccal fat pad removal or reduction is a strictly cosmetic surgery, it’s done to minimize the size of the cheeks for an angular hollow look.

Now why I mention a reduction is for 2 reasons

1- you might not need a full buccal fat removal, only needing a partial for best results.

2- for fear mongers that cope with “but Saar when you age you’ll look shit”
Alright then, just keep it partially.

But it is so easily treatable.
Just a fat graft and skin/neck lift if needed.

TECHNIQUE

The reduction of buccal fat pad is usually performed under local anesthesia with the patient completely awake. The typical approach for removing the buccal fat pad is through an incision in the mouth, towards the back of the oral cavity, near the second upper molar. The buccinator muscle is then encountered and without making an incision, the fibers are separated until the buccal fat is seen. Once the buccal fat is visualized, a thin casing over the fat is opened and the fat is gently teased out while gentle pressure is applied externally. The fat is then cauterized at the base and removed. The remainder of the buccal fat pad then is replaced to its anatomic site and the oral mucosa is sutured close, and the two incisions are sutured. The procedure typically takes about 30 minutes.
