What is the midface ratio?
The midface ratio, often considered ideal at roughly 1:1, measures the balance between the vertical and horizontal dimensions of the middle face, typically from the eyes to the mouth.
How to fix?
A too short or too long midface is usually caused by bad habits in childhood, this causes you to have an either too short or too long midface. It is rarely unfixable.
Elongated Midface - Counter-Clockwise Rotation:
Counterclockwise (CCW) rotation in jaw surgery (orthognathic surgery) is a procedure that advances the upper and lower jaws forward while rotating them upward, typically to correct steep jaw angles. This rotation flattens the jawline, moves the chin forward, improves facial aesthetics, and increases airway space.
Results:
Compact (Too short) Midface - Clockwise Rotation:
Clockwise (CW) rotation in jaw surgery (orthognathic surgery) is a repositioning technique where the maxilla (upper jaw) and mandible (lower jaw) are rotated downwards and backwards, or the back of the jaw is lowered.
Results:
The midface is known to be the most brutal of all the ratios in the face besides ESR/IPD to fix. It is not unfixable and you are 1 procedure away from fixing your midface ratio.

2 - Facial Width To Height Ratio (Ideal: More than 2)
What is the facial width to height ratio?
The Facial Width-to-Height Ratio (FWHR) is an anthropometric measure calculated by dividing the maximum width of the face (cheekbone to cheekbone) by the height of the upper face (mid-brow to upper lip). A higher ratio indicates a broader face, which research links to higher testosterone levels, increased aggression, and perceived dominance.
How to fix?
A combination of jaw implants and zygo (cheekbone) implants can fix FWHR instantly. It is not unfixable.
Results:
Not an unfixable failo although certain greycels tend to try and say its hard and unfixable. Next.

3 - Chin to Philtrum Ratio (Ideal: 2 to 2.25)
What is the chin to philtrum ratio?
The ideal, aesthetically balanced chin-to-philtrum ratio is widely considered to be 1:2 (philtrum to chin), meaning the philtrum length is half the length of the chin.
How to fix?
A bad CTPR is usually either caused by a too long philtrum or a too short chin, meaning you need to either get philtrum shortening or chin implants (both if your chin is also bad)
Too Long Philtrum - Philtrum Shortening
Philtrum shortening surgery, or a lip lift, reduces the distance between the nose and the top of the lip by excising a small strip of skin, usually in a "bullhorn" shape, directly under the nose. By lifting the skin and suturing it upward, it creates a more youthful, voluminous appearance, increases the visibility of the upper teeth, and defines the Cupid's bow.
Results:
Too Short Chin - Chin Implants
Chin implants are artificial, biocompatible materials, often silicone, surgically placed on the jawbone to increase the projection and definition of a weak or receding chin. Super easy fix. Not unfixable at all.

What is canthal tilt?

A positive tilt means the outer corner is higher than the inner, often considered youthful. A negative tilt means the outer corner is lower, associated with a tired or "sad" appearance.
How to fix?
Canthal tilt unlike a lot of other things on here is generally genetic, not caused by something you can control as while developing although there are exceptions. It is however solved instantly with lateral canthoplasty.
Results:
One nice thing to note is that lateral canthoplasty also lengthens PFL therefore being a great 2 in 1 procedure if you also have a bad PFL to go with your bad canthal tilt. It was known as one of the most unfixable failos ever and is to this day. It is not unfixable and is fixed relatively easily, next.

5 - Mouth to Nose Ratio (Ideal: 1.5 to 1.62)
What is the mouth to nose ratio?
An idealized, aesthetically balanced, or "golden ratio" face often suggests that the width of the mouth should be approximately 1.6 times wider than the width of the base of the nose.
How to fix?
This failo, like canthal tilt, is also completely genetic, nothing to do with development. The fix for it usually entails either a widening of the tip and nostrils via a rhinoplasty or widening the mouth via surgery. Rhinoplasty is utilized if the nose is too small (rarely the case, a smaller nose is usually ideal) and mouth widening surgery is utilized if the mouth is too narrow (usually the case here.)
Too Narrow Nose - Rhinoplasty
A customized rhinoplasty is utilized to widen the nostrils and the tip of the nose to a harmonious blend that will go with the mouth's wideness therefore fixing the MTNR.
Results:
Too Narrow Mouth - Mouth Widening Surgery
Mouth widening surgery, or lateral commisuroplasty, typically involves a small, precise incision at the corners of the mouth to release tension and extend the vermilion lining outward. This procedure aims to increase the width of the mouth opening by a few millimeters, often by cutting and restructuring the, orbicularis oris muscle. A bit annoying and complicated, but relatively straightforward. Not unfixable.

What is the bigonial width?
Bigonial width is the distance between the two gonia, which are the most lateral points on the external angle of the mandible (jawbone).
How to fix?
Super straightforward, jaw implants, more specifically a customized jaw implant or a wrap around jaw implant will instantly fix this.
Results:
The dude above went from LTN to chadlite in a single procedure jfl. Whole new life. Nothing is unfixable, next.

7 - Lip Ratio (Ideal: 1.55 - 1.65)
What is the lip ratio?
The ideal aesthetic lip ratio typically features a fuller lower lip compared to the upper lip, often following the "golden ratio" of 1:1.6 (upper to lower).
How to fix?
Super easy. Generally it's fixed through filler but we will not be including fillers in this thread, so we will go with the permanent lip implants.
Results:
Lips are one of the most underrated features in the entire face and even in morphs they are notoriously hard to get perfect, however they aren't unfixable as clearly seen here.

8 - Eye Separation Ratio (Ideal: 0.45 - 0.49)
What is the eye separation ratio?
The eye space ratio, or eye-spacing ratio, is a facial aesthetic metric measuring the distance between the eyes (intercanthal distance) relative to the width of a single eye (palpebral fissure length), typically ideally considered 1:1, or 1:1:1 across the whole face. It helps determine if eyes are considered close-set, average, or wide-set.
How to fix?
This is one of the trickier ones to fix however it is not unfixable by any means.

An ESR is either too wide or too narrow. A combination of zygo (cheek bone) implants and jaw implants can usually be utilized to fix a too wide ESR, and as of today, Orbital Box Osteotomy is the only fix to a too narrow ESR (although very rare to have a really bad one and most people won't notice a slightly narrow ESR.)
Too Wide ESR - Jaw and Zygo Implants
Implants widen the FWHR and make the base of the face larger, therefore making the distance between the eyes seem much closer in reference to the rest of the face.
Results:
Too Narrow ESR (Or a case where the FWHR is good and the ESR is still bad rendering implants useless) - Orbital Box Osteotomy
An orbital box osteotomy is a complex craniofacial surgery that cuts and repositions the bony eye sockets (orbits) to correct severe, close/wide-spaced eyes (hypertelorism) or vertical misalignment (dystopia).
Results:
ESR was known to be the most brutal, unfixable failo out there however it is not the case. ESR is fixable no matter what, technology is advancing super fast and OBO will likely be a widespread procedure within the next decade or so as the demand for it increases as more and more people start wanting it. 9 - Palpebral Fissure Length (Ideal: More than 3.5)
What is the palpebral fissure length?
The palpebral fissure length (PFL) is the horizontal distance between the medial (inner) and lateral (outer) canthi of the eye, typically measuring about 28–30 mm in adults.
How to fix?
As stated previously, a lateral canthoplasty can be utilized to fix pfl significantly, however what if you don't have a bad canthal tilt, just a bad PFL? Well there are multiple routes you can take. Lower eyelid retraction, orbital fat grafting, and orbital implants are generally the most popular as a bad PFL is usually caused by a lack of significant mass to support the upper and lower eye.
Lower Eyelid Retraction
Lower eyelid retraction surgery, or lower lid retraction repair, raises the lower eyelid to its natural position, covering white sclera and reducing dryness or "sad" eye appearance.
Results:
Orbital Fat Grafting
Orbital fat grafting, or fat transfer, is a cosmetic or reconstructive procedure that harvests fat from the body, purifies it, and injects it around the eye socket to restore volume, correct hollowing (tear troughs), and soften bony contours. It often enhances lower blepharoplasty, providing a natural, rejuvenated appearance.
Results:
Orbital Implants (specifically infraorbital implants in this case)

Results:
PFL like ESR was known to be an extremely complicated and unfixable failo, it is not unfixable. Don't get fearmongered, this is actually a pretty easy failo to fix.

What is lower third height?

I made this thread on my phone over a few hours because I was unhappy with the current state of surgery/hardmaxxing threads as they address tons of surgeries nobody cares about. People need to know how to fix their failos, not about specialized skull implants or some other crap.
I've calculated the price of all the surgeries on this list together, and it came out to around 200,000 dollars if you were to need every single one of the surgeries I've stated on this list (including surgeries I counted multiple times because I mentioned them multiple times for different failos.) 200,000 is less than 4 years wage if you make 60,000 a year. Within 4 years if you fully dedicate yourself, you could have a totally new life, you could be reborn. Your life would be perfect. Make the commitment, dedicate yourself fully, and I promise the results will be better than anything you could possibly imagine.
Your perfect life awaits you, your hot wife, your high paying job, the admiration from everyone you meet no matter what, your future kid's respect for you.
