I would go to a derm. They'll check on your liver etc so you don't die accidentally (although this is rare), and you'll know you're getting pharma grade isotretinoin.

Just go to a well known derm depending on the area, and say you've tried other treatments like Tretinoin or CABTREO, maybe benzoyl wash etc. They'll probably prescribe it.

Yes they are reluctant on prescribing isotretinoin, because to them it's like losing a returning customer. Once they hop on accutane and clear their skin they will have no reason to go see the dermatologist anymore.

Some pharmas hand accutane over the counter but this is usually outside the US
