You're bloated and inflamed, fix gut health, go in a carnivore diet that will put you in a ketogenic state. Eat anti inflammatory foods. Hydrocortisone for facial inflammation can help as well.

You have a really good chin

Retatrutide would be de easiest way

You can get it at some Chinese supplier, how old are you

inward gonions
no cheekbones
low bonemass seems to be the main issue here

If your body is lean but your face stays fat, that’s mostly genetics. You can’t target face fat no matter how hard you cut. If it’s above the buccal fat pad, it’s likely midface fat and overall structure, not something you can just melt away. Buccal fat removal is an option but risky and can make you look older fast. Fillers can sometimes create more contour, but they don’t actually remove fat. Fat dissolvers like Kybella can work under the chin but they’re not great for cheeks and can make you look uneven or weird if done wrong. Smile lines usually come from volume loss or repeated movement, most people use fillers to soften them. Redness could be from rosacea, skin irritation, or a weak skin barrier. Try azelaic acid or niacinamide, keep your skincare simple, avoid harsh scrubs or alcohol-heavy stuff. At the end of the day, if your bone structure doesn’t support a lean face, it won’t fully go away naturally. You either live with it, do surgery, or try to camouflage it. No shortcuts.
