how to get rid of stubborn acne between nose and mouth

i have papules and some closed comedones (according to chatgpt) where my moustache grows. ive been to the doctor 3 times, two times they gave me hydrocortisone and once they gave me fucidin and none of it helped. its even starting to spread to the sides of and below my mouth. ive been on 20mg of accutane since around march and i just got back on like a week ago after about a 3 week long break. do i up my accutane dose? any creams i can use?

if its like around the mouth it could be perioral dermatitis and you need antibiotics to fix that

hydrocortisone is a common cause for this so if it became worse after using hydrocortisone id suspect pd
