Not sure if it's bone or fat for my under eye area it's quite hollow compared to the upper eye area. Open to hardmaxxing advice

you have negative canthal tilt, it can only be fixed by surgery

holy grey his tilt is fucking neutral- slightly positive:forcedsmile:

he has lower eyelid droop, should pursue either minor zygomatic filler or a lowwer blepheroplasty, fat grafting for the undereyes

true but it’s an important distinction, the problem is not the tilt, it’s the lower lids and undereye fat pads, if he were to think his problem was nct he'd go for the canthopexy/plasty and could get a sub optimal or uncanny result

thanks for your response, do you think an implant is needed (because of no bone) or is it purely a soft tissue fat thing?

nah all you need is fat there for the undereyes, for your eyelid thats droopy get a blepheroplasty

implant would be ideal but expensive and invasive, it also doesn’t give the same indirect lower lid lift as zygo/cheekbone filler

i'd recommend blepheroplasty for droopy lower lid which creates nct appearance + fat graft (permanent) + optional small zygo filler (needs to be redone twice per year since it will gradually fully fade by the 18 month mark) but cheap, effective, low risk, no recovery time
