Just set the scale , measure the distance and divide them to get the ratio, simple as that nga

Just set any scale mm, cm ,km doesn't matter, for example if you have to measure ESR, set any random scale, measure interpupilary distance you will get a value e.g 4.19, now you will measure biszygomatic width and you will get a value e.g 8.93, now divide them 4.19÷8.93=0.47 , so 47% is your eye seperation ratio

And also choose a photo clicked from 2meters away, and from the rear camera to avoid optical distortion, which can fuck up your ratios
