How to properly plan jaw surgery (DJS, trimax, orthogonatic)

What is IPR ? Yeah they are decompensating the flare, I dont know about the oclussal plane what you mean
IPR makes the teeth smaller so you can pull them back—look it up on Google. The occlusal plane is a straight line from the incisors to the molars, and you can measure its angle relative to the Frankfurt plane
 
The movements seem to be 3mm maxillary advancement but a massive 12 mm of the mandible. That must surely mean significant rotation, right?

It's easier to evaluate this ceph without the flared incisors. I can see why you'd want maybe a marginal advancment of the maxilla and big BSSO movement now. Mandible is clearly retruded.

Will need significant CCW considering disparity of LF and BSSO advancments.
I wouldn't say significant, I would say moderate CCW. 9 - 3 is 6, and he has a significant overjet right now

I would prefer to have a 4 or maybe 5mm advancement, but ramieri said that he doesnt want to do more because it can look weird, he said in the first consultation that 5mm of advancement are the max before looking like a wolf, so i think 4mm might be ideal.

No, I mean he will do a 3 mm maxilla advancement and 9 mm BSSO, so that's a 6 mm difference. Since he has a significant overjet, it won't be a huge rotation.

About CCW numbers I have no idea how it works, but i can tell I might need a big CCW, what do you mean by disparity of LF and BSSO advancements?

It's difficult to explain rotations verbally. In your case, the CCW is going to be pretty moderate because you have a significant overjet.

Much more advancement of lower vs upper jaw.

That's usually the case because if you move the maxilla as far as the jaw it could dogmax, maybe after 5mm, so isnt that something normal, that the lower jaw has much more advancement than the maxilla ?

In Class 2s, there's generally much more mandibular vs maxillary advancement, yes.

No, you have skeletal Class II. Sometimes you need to move the maxilla more than the lower jaw—it's completely case-dependent. The reason why they're moving your lower jaw so much is to correct your overjet. If they moved your upper jaw the same amount, you would still have an overjet

Or if they moved the maxilla further, they have to advance the mandible as well and he'd end up looking like a chimp.

Yes, I do. Chimpmaxed. Was never a candidate for bimax. His occlusion and jaw positions looked absolutely fine to me pre-op.

About the advancement of the jaw i heard that instead of moving the maxilla more they can do CCW to get a bigger advancement without doing risk of overadvancment of the maxilla

CCW moves the anterior (front of) maxilla forward. So there's still a risk of over-advancement.
