Hi everyone,

I’m from France, and I’ve had a hard time convincing dermatologists to prescribe Roaccutane.

Because of this, I’m considering taking it on my own.

I have recurrent acne around my nose that I’ve been struggling to get rid of for a long time. I’ve used tretinoin and adapalene, but nothing has worked so far. I believe that Roaccutane could be the solution.

My skin is still acne-prone and produces a lot of sebum. I also have a bulbous-looking nose, which I would like to reduce (de-bloat).

I would really appreciate advice from someone experienced with Roaccutane regarding how to take it, including dosage and treatment regimen.

Thank you for your help.

start at low dose and up it, like if u are 70 kg start at 40 mg for 1 month and go to 60 mg for the rest, moisturizer lips, skin, hands, and that s it

do liver bloodwork, don t use any topical for acne

i never had liver issues. do i really need to do a bloodwork ? if yes... before or after or during the treatment ?

not necessarily, I have taken isotretoin (30mg for 70kg for 6 months) and I just had bloodwork before and after treatment. My acne is utterly away, just some scars are left. You can still get bloodwork every month, I am not saying that you shouldn't but it is not inherently necessary.

Lab monitoring on isotretinoin is not required if you are an otherwise healthy individual.

For dosing, it is fairly understood that a higher daily dose is correlated with more side effects (dryness/join aches etc) than a lower dose. So if you want to stay at say 20-40mg you can do that, but increasing the dose may have quicker results.

For treatment length, there are two ways to go about it. The first way is "treat-to-clear" where you ignore the daily dose, and you simply clear your skin. Once your skin has cleared up, you go for another 2-3 months. If your skin breaks out at any point during this time, you reset the 2-3 month time.

The other one is cumulative dosing. You take isotretinoin until you reach a cumulative dose of 120-150mg/kg. So if you weigh 70kg, you would take 8400-10500mg of isotretinoin regardless if your skin clears up early on, or later on. Ideally you also leave 1 month of clear skin at the end of treatment as well.

The treat-to-clear method is the preferred one nowadays but it is up to you.

Other than that, make sure you take Omega 3 everyday along side Vitamin D + K2. It is also very important to consume your isotretinoin with a high fat meal in order to increase bioavailability.
