Im going to be using eplerenone for water retention and telemisartan for heart health on cycle

Should I be worried about going hyperkalemic?

Any preventative measures? I was thinking of using a thiazide to offset the excess potassium

Yeah, combining the two absolutely raises hyperkalemia risk. Eplerenone is potassium sparing, and ARBs like telmisartan reduce aldosterone, so together your kidneys hold onto more potassium. The real risk is elevated serum K⁺ --> arrhythmias, especially if kidney function dips. The safest way to manage it is frequent labs, check potassium and creatinine regularly. Using a thiazide to offset K⁺ can work, but it’s not a free pass; you can throw off sodium, volume, or BP if you’re not careful. Diet matters too: watch high potassium foods while on this combo. Basically the bottom line is this defo isn’t something to just wing. With monitoring, you can mitigate risk, but don’t ignore it.
