Happens to me too sometimes, idk why that happens.

It's probably because when you're on a thread and a post says "5 minutes ago" and you wait 5 minutes then someone new posts it wouldn't make sense to have "A moment ago" since the old post would still say "5 minutes ago" even though its been 10 minutes

It's just a bug actually. A little, pesky bug! I have no idea how to get rid of it.
