I'm not an expert but I would say genes.

I mean I know it is genes, but sometimes people who are supposed to have those zygomas haven’t yet developed a strong bone structure due to age or are just bloated to have visible zygomas

Agreed, but let’s say someone is still in the growing phase and is yet to develop a stronger bone structure and sharper facial features, so they might be yet to develop that zygomatic region. Someone like Gandy, who didn’t reach peak bone structure until even 21, and even O’Pry didn’t have that bone structure until his late 20s

Perhaps, I don't think the position changes but possibly the sharpness and the mass
O'Pry looked like a twink before his 20s
