I get horrendous situational anxiety attacks in class

I'm currently 16 and for the past 2 years, I have been getting really bad anxiety in situations where I have to speak up in class, especially class presentations, even though I can easily participate in class and like doing it.

I have no signs of anxiety ever, except for these type of moments. When I was younger, I would literally run class, participate in everything and I would love it.
I still like to participate in class and do it a lot, and teachers would compliment me for it. I don't feel the anxiety nearly as much when I voluntarily participate.

Over the past 2 years, I have literally had worries about the possibility of the teacher announcing a class presentation, or us to have to introduce ourselves.
My anxiety is the worst in settings where I have to wait for my turn and it is not spontaneous. My heart starts beating rapidly and my hands get cold and sweaty, my breathing becomes shallow and I have to take long breaks between sentences and my voice is shaky.

I have never been diagnosed with social anxiety and only started developing it after turning 11/12. In everyday life I'm the complete opposite of what people would call an anxious person. I really like greeting strangers on the sidewalk, and I also like eye contact, with people I know and strangers. Literally no one around me knows I deal with these problems because it is partially normalized, I'm not the only person in my class or old class that gets breathing issues when presenting, but the ones that do, literally do it voluntarily sometimes which is crazy to me, it's like they don't even feel anxious, but just seem like it.

When I am required to do a presentation I literally don't make an effort for it, I just can't. The last two class presentations I had, I literally didn't practice the texts once and I in a group project I would have the group members do everything, but that is just the ways I am and have always been.
The worst part about this whole thing is that I can easily become stressed when there is even the slight possibility of a presentation being announced. It keeps me in a constantly stressed state in school.

Even though every single one of my presentations have been absolutely horrid, no one has ever called me out for it, or even given a clue that it was bad.
I think the teachers have pity for me because I have never actually gotten a bad grade even though none of my presentations were good.

I just care so much about how people perceive me and I can't help it.
My dad has slight social anxiety aswell (I believe), maybe I inherited it or something.

Anyways, I have my next presentation in 3 to 4 weeks and it is gonna be bad. I have to do one about something I can identify myself with, so this is gonna be awful.
We have to do 20 slides 20 seconds each talking freely, which I have never even done before, and I will probably have to practice my text (Spoiler: I'm not gonna do it).

Maybe I should consult a doctor and start taking medications to help it.
