I just had buccal fat removal. I will post the results shortly but I'm a little bruised right now (which is weird because I never saw anyone mention bruising after this procedure).

I was on the fence about it for a long time but holy fuck. I wish I got it so much sooner. Here are my thoughts:

- If (like me) you do not have the bone structure to achieve hollow cheeks at a healthy and maintainable bf%, this surgery is totally worth doing, read my other considerations to decide if its for you

- IMPORTANT: If you lack maxillary width, its likely you have "baby face"), this is when the facial width peaks at your cheeks, and not at the bi-zygoma or bi-gonion. This is because you simply don't have enough tension in the soft tissue, its also NOT something that anterior projection from a bimax can always fix... this is why you see so many underwhelming frontal profile results after bimax, the side ascended hard, but the front stayed virtually the same. Unless you need major downgrafting (like sfs patients who always see great aesthetic gains to their front profile), bimax wont help you in this regard. In my opinion this is a death tier falio, the baby face pill is brutal and anyone who experiences it can attest... buccal fat removal has aged me in the best way possible, I no longer look child like. Also greatly increased the perceived prominence of my zygos, you will see what I mean in the before and afters. Could I have achieved the same or better results with MSE + DJS or implants?? Maybe? But this surgery took 25 minutes and cost nothing in comparison, I'd chose this over the former a million times over.

- If your mandible is overly lacking, this surgery may be problematic for you, as the buccal fat pad provides structure to your face that makes up for your lack of bones, if you remove this, this skin around the area may "collapse", as there isn't enough lateral tension to pull everything tight

Final note: I've said time and again that surgery is an individualistic approach, you need to tailor it to your goals and falios. If you have hypertrophied massaters or your facial width peaks at your bi gonion, then buccal fat removal will make you look uncanny. For me and those in a similar position, it will benefit you. For me it did exactly what I needed it to, while maintaining a natural look, it has not made me look uncanny as people claim this surgery does... although it may not be the same for you. Do your own due diligence.

A point I considered thoroughly before going through. But my conclusion was as follows: I haven't seen enough evidence supporting this IN MEN. Ages like shit in women because it skews their natural female dimorphism by producing a more male dimorphic feature, I don't need to elaborate on this further. For men its the inverse...

From experience, as laxity in the soft tissue and skin develops as we age from collagen and elastin loss / cellular death, the whole face begins to sag. Buccal fat pads NEVER stop this, and often times look worse because here's this big permanent fat pad that is sagging down with the rest of the soft tissue.

The other thing is what the above reply said, EVERYTHING goes bad as you age. Looking marginally better at 55 because I still have my fat pads is not worth the benefit I will experience in my youth from not having them. Its all a case of personal philosophy I suppose. You're within your right to not want this surgery for that reason tho, again just depends on how you think.

Much agreed. I just think its a misconception that buccal fat removal = shit aging. Its such a smooth brain generalized statement. It may even benefit your looks as you age as a man.

Its the weirdest take that buccal fat removal will somehow age you. I've even heard surgeons say it but it makes 0 sense.

Buccal fat does not decrease with age, unlike almost all other facial fat pads. Generally buccal fat gets more prominent with age. Not because it gets bigger but rather because it moves more forward. So removing it is consistent with youthfulness not ageing.

This is especially true in men. Since young men, who have low body fat deplete it, even when they are young enough to still have large amounts of "baby fat".

Yeah I agree that buccal fat removal generally looks like shit in women. Not always but quite often. Not a shocker since they have higher body fat. But its not a function of ageing but rather dimorphism as you point out.

Yeah I also got it done for the same reason even old with “hollow” cheeks look okay I feel like it’s really bad for girls

Yeah I got it back in august, the fearmongering is crazy. My surgeon told me that she personally has done it on people with good and bad bones and that the result never looked weird or uncanny amongst her patients.

I got it and my results turned out pretty good.

Fat grafts, it's a relatively easy fix, though im pretty sure when we're all older like 30-50 years from now more advanced cosmetic stuff will be available in my personal opinion. Though my mom got buccal fat removal young and she still looks good at almost her 60's

only con of bfr is that it works too well and your hollows are too deep looks kinda uncanny when you're very lean very modelesque not a good look in my opinion.
