Positano is real, the film shot the exterior scenes on Ischia island which is pretty similar, though it seems to have less tourists and so was better suited for shooting a movie.

Could be, there are immigrants and other races there now, it's not how it was in 50s-60s when the movie took place - Italy was still recovering after WW2, people were poor, but for Americans it was paradise, everything was dirt cheap, they were considered high status, treated like kings, they could easily get girls, etc. - you won't get now the same experience there like in the old days.
