I went to a craniofacial surgeon who took x rays of my skull and he told me I have a masculine base under my skin. I don’t look very masculine. He doesn’t want to perform surgery on my but suggested buccal fat removal (he will do it or another surgeon). My face has little structure

That I don't know. Whether Barrett has done plastic surgeries or not is an unconcluded debate topic here. Personally I think so, but he still had a great foundation.

Buccal fat removal is pure cope unless you genetically just have really fat cheeks despite being lean. Some people do have this but it’s rare. You need to have some amount of facial fat to look vital. If you look up before/afters you’ll see it usually either makes no difference or makes the person look weird af

Perhaps you are one of the people I described.

But usually if you’re looking to reveal more of your underlying jaw structure, you opt for submental liposuction.
